Gemcitabine (Gemzar®) 1000 mg/m2 en 100 minutes à J1

Oxaliplatine 100 mg/m2 à J2

reprise du cycle à J15


Adaptation de doses

 
Si toxicité non neurologique de grade 3 ou 4 (NCI CTC), attendre la récupération puis réduire ainsi les doses:
    Gemcitabine 800 mg/m2 (en 80 mn)
    Oxaliplatine 85 mg/m2
     
  Arrêt de l'oxaliplatine en cas de neuropathie sensitive ou motrice de grade 3 NCI (hypoesthésie sévère ou paresthésies interférant avec la fonction, ou faiblesse objective avec interférence fonctionnelle).

LV5FU2-Cisplatine

Acide folinique 200 mg/m2 (ou l-folinique 100 mg/m2) en 2 heures à J1 et J2

puis 5-FU bolus 400 mg/m2 en 10 min à J1 et J2

puis 5-FU 600 mg/m2 en perfusion continue de 22 h à J1 et J2 (soit 1200mg/m2 pour les 2 jours)

Cisplatine 50 mg/m2 en 30 mn (précédé et suivi d'une hyperhydratation) à J2

reprise du cycle à J15

Pour pouvoir débuter le cycle, il faut PNN >1500/mm3, Plaquettes >100 000/ mm3 et créatininémie <120 µmol/l
